Finding the forms for existing sheet

The system is running for a few years, and I am new to Smartsheet.

The user will input the form, and those inputs will go to specific sheet. I just want to find out which form it is using. I click on Forms> Manage Forms > No active form is shown.

So, I am wondering how to find the inputs are coming from.
