Formula in form-populated sheet stopped working

I created a UTM-generator for website URLs using the "AI" functionality in Smartsheet.
I'm not going to bother pasting in the formula here because it was generated by Smartsheet based on the need to basically join column values with strings between each cell value. It has been working fine, and there's nothing that can break the formula that's been entered.
Yesterday it suddenly stopped working, so I reversed the order of the form entry so that new rows would be created at the top of the sheet, and it worked again, all day today. Then suddenly it stopped.
Since it has been working flawlessly until it suddenly stops, I have to assume that someone else on my team did something to kill off the formula.
What I'm trying to figure out is how to re-enter the formula or check a cells in the formula column to see if it's still "present" behind any of the other cells. Can I just add a new row and recreate the forumla, or am I going to have to create a whole new sheet and lock out everyone from touching the data in it?

Do you have it set as a column formula?

The column formula option is grayed out. I'm not sure why. It's a simple UTM generator, so there's no reference to unique cells or durations or anything.

I wish I could tell you. It was generated by the AI and once implemented, it disappeared. I have no way to view it now.
Also, this just started happening on the new version I duplicated from the orginal version.
