FEEDBACK REQUESTED - Share Your Forms Use Case

Hi Smartsheet Community!
The Forms team is exploring how YOU use Forms. We'd love your help!
To help us understand your needs, please share your specific use case using the format below. If you have a screenshot or image of the form, please attach it as well.
- Company - Industry
- What is the Form for?
- Who is the audience for this Form?
- What happens after someone submits the Form? (e.g., how are the submissions managed after Forms submissions?)
Example:
- Company - Industry: MBF Corp, marketing agency
- What is the Form for? This form is used by interviewers conducting candidate interviews as part of the hiring process. For example: Does the candidate have experience with XYZ? Yes / No
- Who is the audience for this Form? MBF’s hiring managers
- What happens after someone submits the Form? Once someone submits the form, their answers are recorded in a sheet. This sheet is set up to automatically filter and put the most promising candidates at the top, so MBF can easily review them during the decision-making meeting.
Please share your Forms use case either in this thread or by completing this form by March 14th. Thank you for your participation!

I want the ability to create a form in my 'test' copy of the Smartsheet and then to transfer it to the 'production' copy when I have it dialed in.

I also want the ability to change the font size of the descriptive text below the title of the form, it is currently way too small.

Also, need the ability to add the attachments one at a time (but multiple attachments) so I can require some forms and make others optional with logic, based on what they're asking for.

I use Smartsheet Gov (previously used Smartsheet Commercial at another Company), and the fact that Conditional Logic is not an option in our forms is a HUGE pain point.

I agree with @virgh , having the ability to build Forms (or sheets, reports, dashboards, etc…) and then have them reviewed and tested by a selected few. Then once the testing phase has been verified and completed, push it to production.

Hi All,
I use Forms a fair bit to move selected column data between Sheets.
So, I calculate a form link starting with a target Sheet form URL plus pre-filled (using URL Query Strings) data from selected columns in a source Sheet. When a user in a report clicks and submits the form, Smartsheet transfers that source sheet record's selected column data (and any additional data entered into the form), into the target Sheet as a new (transaction/log) record.
It would be great to have some kind of URL Query String builder, instead of having to use Notepad!
TIP: I surround all the form field/column names and cell data in a SUBSTITUTE([search_text], " ","%20") formula to replace spaces with URL-friendly %20 text, etc.
