Is it possible to create a multiline order form?

Good Day Everyone,

We have a scenario where we want to automate ordering, currently departments have to manually update an excel form, but it would be extremely beneficial if we could create a Smartsheet form to input this information and to then create the form automatically.

I cannot figure out how to create the multiline within a form (Everything in yellow is what we have to capture for each order. Any suggestions

Hi,

I hope you're well and safe!

I developed a solution using two forms (or the same form by using conditional logic) that can be used to submit the primary information and then the other information on a new row for each submission after the first.

Would that work/help?
