Automate sending a Form based on a Date

How can I send a form based on a reminder date? The form will populate the task name and field that is marked a No. I will like the person to be able to change the field in the form to yes and the yes response will update the Smartsheet.

I am not sure I understand your question. Please provide additional information. My goal is for the team not to have to go to the Smartsheet to update the status, but be able to change the value in the form and automate the updates in the Smartsheet.

Hi,

I hope you're well and safe!

There are two options.

1️⃣ You could use an alert that would send based on the date and include a link to the form, and it could be combined with the URL-Query feature to include what task it's for, and then automations or a helper sheet could be used to change the values from the form.

2️⃣ The second, probably easiest option, is using the automated Update Request feature, which is also triggered by the date.
