Form Customization
I was hoping there is a way to tap into the HTML of the forms to customize them a little more. I'm specifically unimpressed with the spacing options. The form looks as though it's made for mobile devices only. Also, is it possible to group questions? Ie all relevant questions enclosed in a box, same background color, etc. Thanks!

There isn't any way to customize your forms. You can however, integrate Google Forms into Smartsheets. Google forms will also allow you to do some great things like additional questions based on your answers.

Hi there,
You can customize your form as well as edit the grouping options by using things like a divider. Please see the two screenshots for "Adding a Field" and "Form Options" (below)

I agree that a few more design options on the form would be great.
I am going to submit a product Enhancement Request with the following:
1- Option of positioning some fields side by side
2- Option to resize the logo at the top of the form (Even if the choices are just "S, M or L)
3- Show the symbols (balls, stars, flags) instead of just the labels. -
To add to your list...
4. Option to split a form into multiple pages (but still add a single row)
5. Option to use HTML tags to emphasize key parts in the field Descriptions and Headers
Note: the use case is we are trying to use Smartsheet for user surveys since having the data in Smartsheet allows us to easily view/analyze it in Power BI. Google Forms isn't a good option since it groups multi-select questions into a single column in Smartsheet - so we'd really prefer to use Smartsheet forms because then we can have 1 column per selection option (then use cross sheet formulas to count the checked items in each column).

You create a pseudo multi-choice question but combining multiple single-choice questions together. I created this example... its not pretty but it will put mulitple optional checkboxes into their own columns.

I had to add a new question for each option and make the question name for additional questions a Space to hide the question name.

Does anyone know if Smartsheet responded with some fixes for adjusting the size of the fields in forms and adding an (IF = yes then add these fields) scenario as described above using google forms?

There isn't a way to adjust the size except with how many rows of text it should be able to fit.
Regarding the IF = yes
Yes there is a Conditional Logic in Forms now for that scenario.

I would also some additional ways to format your form, like creating columns when you have checkbox questions, etc.
