Collecting end of class survey data

I have a sheet with columns for student name | class start date | class end date | instructor. At the end of the class, I need to send a survey form to each instructor regarding each student. I want the responses to be entered on the same sheet and in the same row as the corresponding student row. I've made a form for the survey questions, but the text submitted appears under the existing data. I tried an alternative where I send an update request email but I'm unable to include the questions. Any suggestions on how to keep the survery responses with corresponding student?

If I understand your requirements, you will need to create your own solution using the API.  I have done similar work using the API to update existing rows.

An alternative maybe to create a sheet for each individual teacher populated with the Students and have the teachers work directly with in the sheets.

I can think of a few different ways to set this up.

1. Structure the sheet so that update request would work with the questions.
2. Docusign for collecting the surveys and get it attached to the same row.

Would any of those options work?

You could rename the columns as the actual question. Then it would appear in the update request.

Thanks for the suggestions everyone. I wanted to learn towards sending an update request as well, but I couldn't enter the whole question as the column heading because of the character limit. Fortunately, having the form responses go into it's own smartsheet will work for this project.
