How to create a Questionnaire

I have ten operatives 

I want to ask each of them 53 questions that they will fill in on their smart phones by sending them the link for a web form

how do I do that?

example questions

1    How do you check the phases are concentric?

2    Which of the 7 bands of the HSE/NPL test slide should you be able to see?

3    Explain the term "Kohler Illumination"

4    On what objective should you perform a low power scan?

I would label my columns as "Question 1", "Question 2", "Question 3"..................

Then when I build the form, I would put the actual question in the Help Text field.

Another option could be to use the integration to Google Forms or another third-party solution.
