Is it possible to send an email (with a csv attached) to automatically add rows to my smartsheet?
My internal system has the ability to send csv files via email with latest changes - I want to remove the manual copy-paste inbetween step.

Yes, probably.
Either thru the help of a third-party service like Zapier or similar or the premium add-on, Data Uploader.
Would that work?

I've just downlaoded the trial version, and created a sheet. I then downloaded the app and nothing has synced. Anyone had this issue before? Is it a trial issue, or does Smartsheet typically show issues with syncing between web and desktop app?
