1 Notification/Multiple Approvals

We have a process where associates fill out a form with an individual approval request.  However, they are receiving one email with multiple approvals.  The form was submitted at different times and was approved at different times.   We want one response per request. How do we ensure that this happens? Please advise.Thank you.

It's probably the setting for when it should send the alert.
