How to get rid of 'Report Abuse' and 'Privacy Policy' in a published FORM?

It is very annoying to see these two links on a form. How can I take them off?

    Hi Frank,

     

    Because Smartsheet is hosting the web form, we are required to include our privacy statement and the report abuse links. 

     

    You will notice other web form services such as Survey Monkey and Google Forms also include these links in their web forms.
