Adding a grid to a Form

I'm wondering if there's a way to add a grid to a Form to collect data when multiple names, emails and job titles are submitted. Currently we ask people to submit a PDF form to us that has an embedded table where they list the staff who have taken a training. The table headings are: Name, Email, Job Title. Sometimes they submit one name, sometimes 15. Is there a Smartsheet solution for this? Thank you!

    I'd recommend to either add multiple sections in the form (Name 1, Name 2 and so on) or have a form for one and then reload that form on submission and in that way it can be filled out and submitted over and over again.

    Would any of those options work?

    There are also third-party form options like Google Forms.
