A new user here.

Can someone confirm? Can a FORM be created from an existing sheet, or does the FORM create a sheet, and then additional columns can be added to that sheet?

Forms can be created from a sheet, but a sheet can't be created from a form. Additional columns can be added to the sheet and those can be included in the form(s)
