Autofill of Duplicated Custom Fields with DocuSign

I have created a template in Docusign and connected to my SmartSheet account. I have a custom field for processes covered in the attestation letter that has a corresponding column in my SS list.

When I create the envelope thru Smartsheet Labs, only one of my two custom fields is completed. What am I doing wrong?

I don't feel comfortable posting screen shots b/c of the nature of what I'm working on; however I'll try to lay it out.

Smartsheet Fields: Owner Name, Title, Covered Processes

Docusign Form: Name, Title, Covered Processes (custom field located in 2 places in the Docusign form)

Note: Name & title appear twice in the form as well. Both autofill in both places.

Smartsheet Lab Docusign integration is linked to the appropriate columns. When I create the Docusign envelope to be emailed to the signer, the "Covered Processes" field is only completed once.

I think I've figured it out. I removed my custom field and added individual text fields which I renamed for linking purposes.
