Adding a picture to display in a form

I want to create a form that is a quiz. For some of the questions I want to have a picture where the quiz taker is asked to look at a picture and then answer a question about the picture.  Is there any way to have a picture appear in the custom field of a form?  If not, any ideas on how to link a picture with a field on a form?

You cannot have the form display a picture although it is a great idea. Feel free to submit a Product Enhancement Request.

 

In the meantime, Google Forms has that option and integrates very well with Smartsheet. HERE is some documentation on linking the two together.

Unfortunately we don't use Google Forms.  Any idea whether there are options among Microsoft apps?

Hello! I've had the same question for some time now and have thought of a way to address it. It's not as clean as if the image is in the form itself but does allow you to reference an image on the same page as the form.

1. Create a Dashboard
2. Add Widget: Web Content
3. Insert FORM link
4. Add Widget: Image, Title, Rich Text, etc
5. Publish Dashboard
6. Use the Dashboard link instead of the Form link for others to access

Now you have one page with the form AND the reference images.
