Can someone send examples of a user policy and access provision form that you have developed?

I need to develop a provisioning/deprovisioning form to be used by our internal staff as well as our external vendors that will be uploading data and documents to our new Dynamic View Smartsheet. I've seen one other question similar to mine, but it didn't offer useful or relevant information. I'm hoping someone has had to do this before and could share their information with me.
