Multiple form entries needed; Second entry in form to autopopulate from previous entry

Hello, I have a shipping tracker where each item shipped needs to be on a new row. Since i have people requesting shipping for up to 100 items at once, can i either:

1. Have the form populate their previous entry everytime the they submit the request (that way they only change 1 item and all other column entry's are the same)?
2. Have the option to populate multiple rows in the smartsheet via the form?

Hi,

I developed a solution where you submit one form to add the primary information. Then a second form loads automatically where you can add rows connected to the primary information from the first form, and it reloads for each row.

Would that work?

I am having a similar issue to the one above. I set up a form so that my sales team can request samples for shipment, but sometimes they are requesting multiples items. I want each of the items and request to populate on different lines in the table. Can you share your solution from above, or point to an add on app?
