i am trying to make form in smart sheet like attached image but i am not able to create this please help me on this how can i make this using smart sheet form.

My Question are -

1- How to create custom layout of the form.

2-How to edit html,css of the form.

3-If any option to import prebuilt html as template and use that

main goel to make form using smartsheet tool like attached file ...

To get something similar to that, you’ll have to use a 3rd party service/integration, like Google Forms.

Is that an option?

A possible workaround (depending on the process), you could use a sheet or template instead, and structure is similar to your example.
