Paginate a long form

Hello community, I'm new to Smartsheet and have a few questions for using the Forms.

I have a survey questionnaire with 20 questions.

Is there a pagination feature in Smartsheet that lets me break the long form into several pages? It's a common feature in mainstream survey tools like Qualtrics or SurveyMonkey.

Is there a capability in the Smartsheet form that lets the respondent to upload a document? Something like an Attachment button on the form.

Welcome to the Community and the wonderful world of Smartsheet!

Unfortunately, it's not possible at the moment to have pages in the form, but it's an excellent idea!

Please submit an Enhancement Request when you have a moment

As a possible workaround, you could use the Conditional Logic feature or a 3rd party integration like Google Forms or similar.

Yes, there is the possibility to add an Attachment Button.

I have a form with over 50 fields... I broke them up into categories and used a set of checkbox "helper columns" to show/hide the sections with the form logic.

basically set up your columns

Then put the checkbox column (in this case "Category1") just above the group of fields that you want to show/hide with that checkbox. Add the logic of "if checked" (only option for checkboxes) to Category1. Tell it to show the fields when checked.

Even though the end result of the form is a mile long, when the user first sees the form, it is not as daunting.

@Ezra would you be able to share what you did? I'm trying to take a sales form and put it into Smartsheet so we can report off the data. However some questions have multiple layers (if yes then ask another question) and I can't seem to figure out what is the best way to do this in SS.

Is there any way to send a follow-up form to a user once they complete the first page? Something with an automation workflow?

I recommend starting with a form and then continuing with Update Requests.
