Copy form to another sheet

Hi team - is there any way to copy a form you use on one sheet to use on another sheet (with exactly the same fields, just another sheet name)?

I have a form that will have over 50 questions and help text and it would be great to use that form on other sheets that require an intake form.

I looked for a "save as" option on the form but I did not see that option.

It's been my experience that when I use the "save as new" option under the "file" dropdown for the sheet that the form associated with the sheet is also copied over.

What if you want to use an existing form on a sheet that already exists (made previously from the same template but before the form was created)?
