How to collect global data for 250+ countries?

Attempting to collect data, from multiple offices across the globe, to show how many locations a company has in various countries. Currently, there are 256 countries and roughly 5 "details" fields. We were hoping to be able to collect via a form; however, we have not had any luck. Any suggestions on best way to collect? I have attached an example of the type of file currently being distributed via Excel. Thank you!

Why doesn't the form work?

Another method I'd recommend would be to send out update requests instead.
