Enhancement Request - Multiple Rows From a Single Form Submission - Multiple Lines

Having the ability to create multiple line item entries on a single form. Meaning, let's say you create "header" information 1x on a form, but then enter multiple Line Items (which end up as separate rows) rather than having them extend into multiple columns. Data is not user friendly by any means in this current format, and limits the dynamic of the use of forms. Forms is still great, but this would be a tremendously helpful addition to save time on re-entering header information on a form, such as name, reference numbers, need dates, etc.

As a possible workaround, I developed a solution using two forms (or the same form by using conditional logic) that can be used to submit the main information and then the other information on a new row for each submission after the first.

Would that work/help?

Appreciate the reply and suggestion. I'm not sure, but I'm guessing that in your workaround, after you enter the header information, that each time you enter the line item it clears out and you can't see what you just submitted, and then you submit the next line item. It'd be ideal that while a person is entering their list of items needed, they can see them as a review before submitting the form. Below is a breakdown of what I'm envisioning for a solution. Smartsheets does exactly what I need...but for one single line item and I need duplicate line items in rows, but filled out on a single form at a single instance. For now, I'm assuming this isn't possible in current Forms, and will pursue alternative solutions. Problem is finding the dynamic form solution, cheap. GoCanvas/etc. have the solutions but they come a relatively hefty price in comparison.
