Send a notification 3 weeks after a certain date

I have a date column within my sheet, and I want to send out a notification when it has been 3 weeks after that date. This is what I have setup:

(I am the owner of the sheet, so I know permissions are okay)

I am the contact in the PBM cell, and I confirmed the email address is correct. I also created a test and entered in a date 3 weeks ago into the date column, but I never got a notification.

Anyone know what I am missing?

You're missing a Condition clause. Your Trigger is when you want to run the automation (in this example maybe daily) and your Condition is under what condition is the message going to go out. In your case it's when the Date Updated is past the last 30 days. So your automation needs to look something like this:
