Purchase Order with multiple Items/Rows (in a Form)

How do I build a Purchase Order ( using form) that will allow the user to add more rows ( items ) in the Purchase Order.

I have a field that says "Add more Item" and the response is a single select dropdown Y/N

I've tried to use the logic, but the only option is to "show fields" after I enter a response to my condition.

I developed a solution using two forms (or the same form by using conditional logic) that can be used to submit the main information and then the other information on a new row for each submission after the first.

Would that work/help?
