Form question - Possible App

Hello, I have built a form that we will be using for self checkin at our Health System for COVID19 where the employee will log their employee number, temperature, and answer the COVID19 questions before they begin their day. I would like to be able to have a App Shortcut to put on their mobile devices I know I can walk them through downloading the app and how to search for forms through the app but would like to be able to just have a shortcut basically on their phones that look like an app. Has anyone done this and do I need to purchase an add on from Smartsheet to do this? Any suggestions would be greatly appreciated. Thank you in advance for your help.
