Conditional formatting with AND not working as expected

I've created a conditional format using and that states if the date in a column is in the next three days, and the column next to it isn't yet filled, make the cell yellow:

As you can see below, it's filling in yellow even when that "send to copy editor: actual" is NOT blank:

Any recommendations?

Keep in mind that the rules are in order from the top to bottom.

Hello, Andrée!

It's close, but not quite working. The very first condition states that if "sent to copy editor: estimate" is in the next three days AND "sent to copy editor: actual" is blank, then fill with yellow.

Lines 89-93 are examples where that rule is being broken because "sent to copy editor: actual" is NOT blank but they are still yellow. The expected result there is that they would not be filled.

I just had just isolated the wrong column to highlight in some of the instances of the conditional formatting. Rookie mistake!
