Placement of Form Elements

Is it possible to setup your form elements like in the attachment below?

It seems that you can only "stack" form elements on top of each other. This will make the form very long. I would like the flexibility to place form elements into a grid. What are my options?

Smartsheet only allows for vertical stacking. You may need to look into a 3rd party form. I am not sure if Google forms allows for horizontal placement, but I do know that it integrates very well with Smartsheet.
