Attach a pdf to worksheet and automate the forwarding of that form

Well, as the question suggests, I was hoping to attach a pdf to a form and after submittal have that form forwarded to another person via a workflow.

Is this possible?

You could create automation that sends an alert to the other person (presumably, they'd be in a contact column? Or always the same person?) when a new row is created, and you can set that alert to only send selected fields, including the attachment:
