Leveraging Forms for Auditing
Help!
I'm looking to use Forms as an auditing tool for our team. Here's the use case:
When we're working with providers, we need to track when they meet all of the necessary criteria. We've been using a Google form with radio buttons:
- How do I replicate this type of form in Smartsheet?
- The biggest issue is when we have multiple providers! How do I capture in one form this type of field for each provider? The data helps us to know how many errors we have that need to be addressed.
Any workarounds/thoughts would be greatly appreciated!

My suggestion would be one submission per provider. To get the radio buttons similar to the above screenshot, you would need to create a separate column for each criteria and use a dropdown type column with "Correct" and "Incorrect' as the options within the column properties. You can then configure the form to display each of the fields (columns) as radio buttons.
