Has anyone created a Lesson's learned template

Hi Everyone,

Looking for some sample Lesson's learned collection worksheets. Does anyone have anything that has worked well? Are you willing to share?

I am actually in the process of creating one myself. What I found was that using a Form to populate my data got me the results, it was the only way I could sort, should I have multiply lesson learned for 1 project. And on the plus side make Data entry super easy.

Also wondering if anyone has already started a lessons learned template or form that could be used to inform a workshop post project completion?
