Fields not available for form creation

I started a sheet (let's call it sheet1), used "Save as new" to save it to sheet2, added columns and made some changes. I want to create a form using the sheet2 columns, but the new columns I added are not showing as available fields.

Hi Andree -

Problem solved! I selected "remove all" fields in the form then selected "add all" and all fields showed up. Apparently once has to add all to be able to see the newly added fields.
