Form to create child rows?

I am new to smartsheet and have found that a form can't be used to auto-generate child rows when a parent is created, BUT can a form be used to directly generate a child row?

For example, I have a sheet with a list of circuits. Currently I have reports that generate from information manually populated as a child rows. That way I can track circuit outages for example.

I would like a form where you picked from a list of existing circuits, then filled out the appropriate information and it would create a new child row under the parent you picked from a list. This sounds way outside smartsheets ability as I'm learning, but thought I would throw it out there. Ultimately I would like to have other employees have the ability to add this information without extensive training and unleashing them on this sheet.

I'm not opposed to using a new sheet, but would still need to somehow pick from a list of circuits in the form.

    I developed a solution using two forms (or the same form by using conditional logic) that can be used to submit the main information and then the other information on a new row for each submission after the first.

    Would that work/help?
