Forms 2 part Question

Ive created a form but every time my user attempts to add a record, it adds additional blank rows to my master sheet.

Also when they are successfully adding a record sometimes it throws the new record about 100 lines below the last row used.

Has anyone had this problem and any idea how to fix?

Have you looked In the Activity Log for clues?
