I'm trying to come up with a similar format that will make it easy to tabulate results. I know I can make something work, but wondering if there is a forms feature that allows for collecting survey responses in this type of format:

We do a survey that is similar, and use single-select dropdowns for each question (with Very Satisfied, Satisfied, Neutral, Slightly Dissatisfied, Very Dissatisfied). Each question is a column, and the form then enters each completed survey as a new row.
I then have columns added in with IF formulas to give point values, which allows me to do some reporting and summarizing, which we end up using on a dashboard.

I have a filter set so that if this task complete box is checked, then the row is hidden. Is there a way to make it so that if the parent row is checked, then the child rows hide as well? Right now when a parent row is checked, only the parent row is hidden.
