Workflow emails sending to the same user 3 times

Hi
I have an issue with my workflows where the same user is being sent the confirmation email 3/4 times per run through.
Any help would be greatly appreciated. The 'otherwise' alert someone email is the one that is being sent multiple times.
The workflow is almost the same for the other 3 workflows, however the trigger is adjusted.

FYI I am having the same issue I can't seem to resolve it. Here is what I read to understand why this is happening:
- If approval columns and values are re-used across workflows, you may receive duplicate alerts. To prevent this, use different approval values or add conditions after each approved or declined request.
This function does not make sense and I am trying to see what it talks about when it says columns and values. If you figure it out please share.
