Can QR code be used to connect a random user to a form(survey) in smartsheets?

We are wanting to provide a way that when we engage with clients, they can scan a QR code to leave feedback about their customer service experience after face to face engagements. I am wondering if there is a way to use QR codes to link and individual to a "form" that can be used to collect feedback, then track all the feedback in smartsheets?

Yes, you can create a QR code and link it to the URL of a form.
