Is this functionality available in Forms?

I have a sheet of sheet of user information that I need to add to and want to be able to do so with a form. My hope is to be able to create form with a drop down that allows individuals to pick their name or email address from the drop down (that will correspond to the row on the sheet) and then they can complete the other questions on the form that will then populate for that row for that individual. For example, does each user need a new keyboard, monitor or mouse (those are three columns in the sheet that would be populated if they select them on the form). Is this something that can be done?

Any help would be greatly appreciated!!!!

I developed a solution using two forms (or the same form by using conditional logic) that can be used to submit the main information and then the other information on a new row for each submission after the first.

Would that work/help?
