How to convert value from different cell into date format

Thank you in advance for your advice.
I like to convert values from the different cells into date format to capture the follow-up date in a report.
How do I input the new formula in "Date tp follow up"

Try something like this.
=DATE([D.O.B Year]@row, [D.O.B Month]@row, [D.O.B Day]@row)
