Is it possible to set up a real-time time based automation workflow for blank cells?

Our Intake Clerks input new referrals to be worked up via a form. The insurance department subsequently verifies insurance and inputs the confirmed insurance information in the "Insurance Information" column. We would like to have some real time insight into how long from when a referral is submitted and populating a row in the sheet the insurance information remains blank for (ie referral entered and populated at 10:00AM, an alert goes out half our later if the "Insurance Information" field is still blank). Attached is the closest thing I could think of, but it is not real time and would only pick up on the cells that are blank at that particular time and would not show how long they were blank for. Thank you in advance!

You could use the record a date feature, but you'll only get the date.

Here's a possible workaround or workarounds
