Can we merge two form inputs into single row?

I have a sheet of 10 columns. I have created a form out of it. First 5 needs to be filled by one person initially. However, remaining 5 fields needs to be filled by other persons (multiple persons) at same time and also at other times in some cases. How should I fix this?

What is the best solution for this?

I'd recommend using either a report or the update request feature.
