Formula for referencing two columns in another sheet.

Hello,

I would like to add a formula for the partners' cell(image #1)

to reference the two columns in another sheet (image #2).

The current formula you see in the first image only references the Partners(existing) column.

Any help is deeply appreciated!

You would want to create a new column in the source sheet that combines the two columns into one and then reference that.
