Suggestions for using a form to get a list of names and email addresses

We have a training request form that 3rd parties use to request access to a training program for their organization. We want to extend the form to require the person submitting the request to provide names and email addresses for the people that would attend the course. Each person listed would receive another form used to capture additional information about the participants. There is no limit on the number of people that an organization can send to the training if they are approved.

I developed a solution using two forms (or the same form by using conditional logic) that can be used to submit the main information and then the other information on a new row for each submission after the first.
