how to deleted extra rows from source form

I have created a request form which I linked to out JIRA to create a support ticket.

The form appeared as a table in the description part of JIRA but with extra columns. please see image below, I want to delete those from the source form. Can I do that?

Since this has to do with the appearance in JIRA, I would recommend posting in the Atlassian Community where you can talk to other JIRA users.
