Columns (fields) not available when creating a form

I am creating a form from a sheet but not all the columns in my sheet are available to me in the form.

I've tried making a copy of the sheet and creating new forms but I still only get a subset of my columns.

What could be causing that?

Column Formula fields will not be available in a form. Is it possible the fields you are missing contain column formulas?

Are calculated fields not available in forms? That could be why I'm not seeing them.

If your column has a column formula it will not show in forms (cell formulas are allowed)
