Form is not working

We have a lot of forms that get used daily, but we are having random problems with this one sheet/form, a couple different things will happen and it is at random:

1) if two forms are uploaded close to one another, only the first one will come in and the second one will not submit and it gets lost.

2) They will upload one and it will get uploaded twice.

3) Sometimes they will not upload at all even though they will get confirmation that it was successfully  submitted.

All of them get submitted to the top of the sheet, there is no hiden rows/columns, and no filters are on. What can be fixed?

Are the forms being submitted by the same user or different users? Are different users using the same login as each other on different computers?

Are you using Chrome or new Edge browsers? Are they fully updated?

Have you tried recreating the sheet and form?

they are being submitted by different users, all using phones and/or on desktop on Safari/Google Chrome

We have not recreated the sheet, but have recreated the form

@Haylee It may be easier to just recreate the sheet and form and see if the problem still happens. If it doesn't, chalk it up to "something got wonky" with the first sheet. If it does, then you have something to go on, that it may be a setting or a particular of the columns or something.
