Multiple Entries being incorrectly created via Web Forms
We are experiencing intermittent issues with the same entries (tickets) being logged via a web form:
#1 Multiple entries being logged a few minutes apart. User is adamant that only 1 ticket was created.
#2 When this occurs, the created time & date stamp is sometimes 12+hrs after the time when the form was actually completed.
This has happened twice now in the past 12 months and I am unable to pinpoint any common aspects.
Has anyone else experienced this problem

I checked the activity log and all the double entries appear valid as though they were just entered multiple times. The only odd aspect is that the time stamp is much later (+12hrs) than when the user claimed they logged the ticket.
