Why is my FORM not correctly populating the Sheet?

Hi, I have a sheet that is not being correctly populated after the FORM is submitted. The Sheet is only populating the comments columns and not the columns where you choose a color. How can I fix this? Also, there are no filters for this sheet and the data for the columns where you choose a color are not populating elsewhere. The FORM is also designed to populate the sheet with a new row at the top.

Hi Andree, Here is the example. When I fill out the FORM for this sheet, the comments boxes populate. However, the boxes next to the comments boxes do not populate with the color that was selected while filling out the FORM. I put the color that it was supposed to be in the comments. As you can see those columns are blank.

Do you have formulas in the columns next to the comments columns?

There aren't any formulas. The blank columns are selected from the FORM. Here is a screenshot from the FORM. This color then is supposed to populate into the sheet

What type of column is it, and do you want it to show as a symbol or a color using Conditional Formatting?

It should show up as a green or red bubble. So the column is set up as a symbol.
