Form submitted, not populating sheet

Hello Smartsheet Community,

One of our forms seems to have stopped working. The form is successfully submitted but it doesn't populate the sheet, it's not hidden anywhere i.e. at the very bottom and I've created multiple forms and it's still not working. I've also tested another form we use for a different sheet (same process) which works fine.

I've checked activity since the last form was submitted and although columns have been deleted, they're not linked to the ones on the form or what information needs to be on the sheet.

I'm at a complete loss as to what's happened and need to solve this quickly as we're at a standstill and use this form on a very regular basis.

Is this still happening? My form has stopped working. I get a confirmation email with the info but the info does NOT go to the sheet.

Hello - Are there updates to this issue?

Though most of my sheets are working properly, I was notified of a submission that never made it to the sheet. I've reviewed the activity log and confirmed this record did not populate at the bottom of the sheet or in a different text color. I ran a test submission and was successful, but am unable to explain what happened with the previous entry that the activity log also validates did not populate on the sheet.
