The Confirmation e-mail has the wrong title. How do I fix this?
Note: I have tried creating a new form, but the problem persists when I select send me a copy of my response

I tested your title in my form and got the same error.
I also tested an automated Workflow with an Alert that would trigger for each submission and that worked, so I think it's a bug in the Form feature.
You could set up a Workflow with an Alert instead in the sheet that would automatically send the submitter a copy.
I'd recommend testing that as a potential workaround.

Where in the application can I make the change to the title? I can't seem to find anywhere to make the change.

You'd change the Form Title inside the form. (see below)
Strangely, the title is correct, but the email sent by checking the Send me a copy of my responses is incorrect in the email.
Note: I did open a ticket with the Smartsheet Support Team.
Is there a way to edit the email that is sent? Unfortunately, I can't locate anywhere to change the phrasing.
