Fields Will NOT Add to Form After Completing Formula

Hey Leibel, I am now being asked to add another field to the form, can you please help me figure this out?
The field name is "OPPTY #" and it will have a "OPPTY-12345" in the text. Can you please let me know how I would add this to the form? A screenshot of the format is below.
Any help is appreciated!
